Name the organizations involved in your own plan. The website operator, clinical practice, and pharmacy may be separate businesses. A patient should know which one is responsible for a medical decision, which holds the account, and which handles the medication.
Read the price in its proper context
The FAQ lists a $299 monthly self-pay plan covering clinician and dietitian appointments, messaging, and platform access. Medicines and laboratory work are additional. Insurance participation does not eliminate a patient’s copay or deductible; ask for the cost under your specific plan rather than treating the self-pay figure as universal.

Find the cancellation boundary
A cancellation route is linked from Form’s site, but ending future participation should be distinguished from bills for care already delivered. Ask when access ends, whether another appointment is scheduled, and how any outside pharmacy refill will be handled. Keep the financial request separate from clinical stopping instructions.
A useful request names the next renewal and the current order separately. Ask for confirmation of both the effective cancellation date and the pharmacy status. Instructions for stopping or changing the medicine itself need to come from the treating professional.
Identify the actual prescription
Form’s FAQ states that its clinicians prescribe FDA-approved medications. The clinician still decides whether a medicine is appropriate, and the patient’s insurance determines coverage. A listed brand is not proof that every patient qualifies or that a prior authorization will succeed.
FDA approval concerns a particular drug product. A compounded preparation has not received that approval, and evidence for a different formulation cannot guarantee an individual result. Read the product-approval explanation and ask questions about the exact medicine proposed.
